July 7 or Later: Prepare for the Delayed Start Date for Fellowship Training and Orientation Activity

The National Association of Designated Institutional Officials (NADIO) has formally recommended that by 2027, all fellowship start dates and orientation activity are delayed to July 7 or after.

The July 1 fellowship start date has been a long-standing complication for internal medicine. NADIO’s recommendation addresses challenges that have plagued the residency-fellowship transition:

  • Training and education disruption due to fellowship program requirement that rising fellows report to fellowship prior to completing residency training
  • Increased pressure for fellows to adjust to a new environment in a compressed timeframe
  • Potential violation of federal labor laws or CMS funding rules
  • Administrative encumbrances related to scheduling clinical coverage to manage the gaps

Departments of internal medicine and GME programs vary broadly and there is no one-size-fits-all approach. Therefore, AAIM urges the GME community to work directly with their respective DIOs and institutions to strategize and operationalize the delayed fellowship start date by July 2027 (FY 2028).
