Demystifying Milestones 2.0 – CCC Faculty Development for Success

The Next Accreditation System integrated use of milestones in trainee assessment in 2013. These milestones served to define the trajectory of development in competency-based medical education. ACGME assured programs of plans to periodically review and revise the milestones to remain relevant and useful to each specialty. Problems with the first milestones as reported back to ACGME included too many subcompetencies, complex and negatively-worded language (“edu-speak”), too many milestones per subcompetency and lack of consistency across non-Patient Care and Medical Knowledge subcompetencies. The new Internal Medicine Milestones 2.0 was published in 2020 after incorporating programmatic and individual feedback. While the new iteration addresses most issues of the initial milestones, the issue of consistency remains. Appropriate understanding of the milestones by faculty assessors ensures graduation of competent clinicians.
Through interactive group and case-based activities, the webinar will delineate the salient features of milestones competencies. Presenters will include a competency that is more objective (medical knowledge) and one that is historically nebulous (professionalism) to highlight how each has complexities ripe for discussion. The goal is to foment a shared mental model, develop a shared vocabulary, and troubleshoot complex learner cases. The ultimate goal is to not only introduce a practical approach, but to inspire and inform individual faculty assessment practices, while also serving as a basis for future programmatic/CCC faculty development webinars at other institutions.
