Back to the Bedside… With a Needle: Building a Flexible Competency-Based Bedside Procedure Curriculum

The American Board of Internal Medicine (ABIM) drastically changed the requirements for resident training in bedside procedures in 2019, shifting from a required core set of procedures to a requirement that “Residents must be able to perform all medical, diagnostic, and surgical procedures considered essential for the area of practice.” With increasing reliance on specialty services, evolving standards around ultrasound guidance, and work compression, it has become more difficult to achieve the desired competency through traditional clinical training alone. This is a significant problem for residents preparing for fellowship or independent practice. Fellowship directors have noted that fellows enter their programs with less experience in bedside procedures, which has led to some specialties eliminating some procedural expectations, such as central line placement for dialysis in nephrology fellowship programs.

To better prepare residents for their unique careers it is critical to leverage simulation training and individualized clinical opportunities through a competency-based curriculum. In this webinar, the presenters will discuss experience building bedside procedure curricula at each of their institutions. The presenters will address the common barriers to pairing simulation with the relevant clinical experience and will review best practices for developing and maintaining a specialized group of faculty to train and assess resident procedure skills, along with strategies for implementing evidence-based assessment methods that inform competency-based determinations for privileging including the use of procedure competency committees.
