Registrations received after Friday, February 8, 2008, are subject to a $75 late fee. Registrations received after Friday, March 21, 2007, will be considered onsite registration and will be subject to a $100 onsite registration fee.
APDIM accepts payments by check, MasterCard, and Visa. Purchase orders are not accepted for registration payment. Payments made by check must be sent with a copy of the registration confirmation email to ensure timely processing.
All refund requests must be submitted by Friday, March 7, 2008.
APDIM uses the registration fees to cover the cost of food and beverage (breakfasts, breaks, lunches, receptions), audiovisual equipment rental and technical support, room rental, speaker reimbursement, continuing medical education accreditation, the online registration system, course material duplication, expenses related to planning efforts, and staff efforts to support the planning and execution of the spring conference.
