Leaders in medical education may feel powerless in fostering a positive educational culture, but defined leadership behaviors can create and promote healthy both leadership and followership environments. Alternatively, and unfortunately, there are well-defined characteristics and behaviors of leaders that can promulgate toxic environments hampering learning, resident and faculty well-being, and patient care.

 

While the clinical learning environment is complex in its operation, there is a straightforward role for individuals, institutions, and leaders in defining the quality of the “cultural milieu” in which medical education and patient care occurs. The Institute for Healthcare Improvement has proposed strategies for promoting “joy in work” and outlining clear roles for leadership. Others have called the avoidance of toxic leadership practices a “must.” Moreover, effective leadership can decrease burnout and improve morale within organizations.
